This form is for use by members of our committees and ministries. Please print the check request, fully complete the form (including authorized signature), attach supporting documentation and forward all to our Financial Secretary. Remember, we are a tax-exempt organization - contact the office for a tax exempt card prior to making your purchase. We cannot reimburse state tax paid for church-related purchases. Thank you.
